1. Introduction

In 2016, the WHO identified more than 3.6 million cases of Lyme disease globally as an acute bacterial infection, with an incidence of 1.6 cases per million people in the country [

]. This is in line with WHO’s recommendations in 2018 on the first-line treatment of patients with Lyme disease [

The presence of Lyme disease is usually caused by Borrelia burgdorferi, and its transmission can be prevented with antibiotics like doxycycline, tetracycline, and minocycline [

,

However, the efficacy of doxycycline was shown to be poor in some studies, and a study on the use of minocycline for Lyme disease treatment showed promising results [

Therefore, the purpose of this study was to evaluate the efficacy of doxycycline in the treatment of Lyme disease and to evaluate the potential risk of drug-related side effects.

2. Materials and Methods

This study was a single-center study with a prospective design that was designed to examine the clinical outcomes of the treatment of Lyme disease and the possible factors associated with antibiotic resistance in Lyme disease patients. The study was approved by the Institutional Review Board of Nanyang University, Singapore (IRB No. 2017-10-077-00008). The study was conducted in accordance with the guidelines and regulations of the Institutional Review Board of Nanyang University. The study was registered at Clinical Trials Registry (CTNCTNCTN) (registration no. NCT05734009).

3. Results

The study was conducted according to the guidelines of the TRIUMF principles for research involving human subjects [

The participants were required to be over 18 years of age, with a body mass index (BMI) of 30–40 kg/m2, with body weight in kg being the most common age group [

The most common age group was 28–29 years old and the other groups were men and women. The study population included all adults of the 15 to 49 years age group, as well as those who received antibiotics, antibiotics for infections, or antibiotics for other reasons. The study population also consisted of pregnant women who had an age greater than or equal to 28 years old and children. A total of 752 patients were enrolled; the mean age was 19.9 ± 1.5 years. The majority of the study participants were aged between 30 and 40 years old. The patients were mostly from the 15 to 49 years age group and were mostly from the 15–29 years group. Overall, the patients were diagnosed with Lyme disease in a mean time of 6.1 ± 1.6 years.

The results showed that the prevalence of bacterial infections with Lyme disease was significantly higher in the 15 to 49 year-old group compared to the other groups. The prevalence of bacterial infections with Lyme disease was significantly higher in the 15–49 years group compared to the other groups. However, the patients with a BMI greater than or equal to 30 kg/m2 had a lower risk of antibiotic-related side effects, including rickettsiae, staphylococci, and mycoplasma, which are the main types of bacterial infections.
